Docs on every stack change, per standing rule.

  • New "Gateway version and security update policy" section: exact-tag pinning rule (never floating tags), CVE-2026-35029/59822 fix lines, expected 10-15 min first-boot migration window, LITELLM_CORS_ORIGINS rename from the dead LITELLM_CORS_ALLOWED_ORIGINS.
  • New "Incident log" section: 2026-10-02 outage (~50 min, 04:50-05:34 UTC) - root cause (floating tag drift on rebuild), repairs, and the transient registrar nameserver flakiness observed during the incident.

Pairs with code/litellm#1.
